Welcome back! We are blessed to have you continue as part of our Circle of Love Family! The following is a list of all the forms we need you to complete for your child's student file for September. The following Application Forms are available for download and printing.
If any of these forms are not downloading properly, please see the school office and we can provide you with a copy.
Please note that email is the most used method of communication with our families. Please include your email on the Permission for Directory Information. If you do not receive any email at the beginning of the school year, please come in the office to verify your email address.

Admission Package
We are excited to have you join our Circle of Love family! Enclosed are all the forms we need you to complete for your child's student file. We also would like a family photo (and it can be as simple as a color copy of a snap snot.) Below is a check list of every form that is needed. If you have any questions, please call the school office at (310) 545-5653, ext. 221.

OPTIONAL:
(If you have any questions or concerns about volunteering in your child's/grandchild's/etc. classroom, please contact the school office to make sure we have your volunteer paperwork in order. Copies of the intended volunteer's immunization records are required along with the Volunteer Form.)

If you are interested in becoming a Room Parent, Trike-A-Thon Chair, Enrichment Coordinator, or any other volunteer position, please fill out this form and return it to the Circle of Love Office!
IMPORTANT NOTE:
Under a new law enacted by SB 277, beginning January 1, 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into child care or school in California. For more information go to https://www.shotsforschool.org/.
